Could It Be a UTI? Check Your Symptoms

UTI symptoms can range from mild to quite severe. Tap each symptom below to understand what it means and whether you should come in for assessment.

Burning or stinging when you urinate

A burning or stinging sensation when passing urine is the most recognisable symptom of a UTI. It is caused by the inflammation of the bladder and urethra as the infection irritates the lining of the urinary tract.

Needing to urinate more frequently than usual

Feeling a frequent or urgent need to pass urine, often producing only a small amount each time, is a hallmark of a lower UTI. The bladder becomes irritated by the infection and sends signals that it is full even when it is not.

Cloudy, dark, or strong-smelling urine

Changes in the appearance or smell of urine can indicate bacterial infection. Cloudy urine often contains white blood cells and bacteria. Strong-smelling or unusually dark urine alongside other UTI symptoms warrants assessment.

Pain or pressure in the lower abdomen or pelvis

A dull ache, pressure, or cramping sensation in the lower abdomen is commonly associated with bladder infections. This discomfort is caused by the inflamed bladder wall and is distinct from the sharp pain of a kidney infection.

Blood in the urine

Pink, red, or cola-coloured urine can indicate blood in the urine (haematuria), which can occur in UTIs due to bladder wall inflammation. While this is often associated with a straightforward UTI, your pharmacist will assess whether further investigation is needed.

Feeling generally unwell, feverish, or shivery

A mild temperature can accompany a bladder infection. However, a high fever, chills, back or side pain, or nausea alongside urinary symptoms may indicate that the infection has spread to the kidneys. This is a more serious condition known as pyelonephritis and requires urgent medical attention.

Seek urgent help if: You have a high fever, back or loin pain, nausea or vomiting, rigours (uncontrollable shivering), or feel very unwell. These are signs of a possible kidney infection (pyelonephritis) and require urgent assessment beyond the Pharmacy First pathway. Call 999 or go to A&E if symptoms are severe.

What Is a Urinary Tract Infection?

A urinary tract infection is a bacterial infection affecting any part of the urinary system, most commonly the bladder (cystitis) and urethra. UTIs are significantly more common in women than in men due to the shorter length of the female urethra, which makes it easier for bacteria to reach the bladder.

The most common cause is Escherichia coli (E. coli), a bacterium found in the digestive tract that can transfer to the urinary tract. Most uncomplicated UTIs in otherwise healthy women respond well to a short course of antibiotic treatment.

It is worth noting that some conditions, including certain sexually transmitted infections such as chlamydia, can cause symptoms similar to a UTI. Your pharmacist will consider this as part of the consultation and will refer you appropriately if another cause appears more likely.

The NHS Pharmacy First service covers uncomplicated lower urinary tract infections in women aged 16 to 64 who are not pregnant and do not have conditions that make them higher risk. If you fall outside this group, your pharmacist will refer you appropriately.

This pathway is available through the NHS Pharmacy First service for women aged 16 to 64 with an uncomplicated lower UTI.

Women who are pregnant, under 16 or over 64, or who have conditions such as diabetes, immunosuppression, or recurrent UTIs requiring specialist input will be referred to their GP. Men with UTI symptoms should also see their GP, as UTIs in men require further investigation.

Your pharmacist will take a full symptom history and may use a urine dipstick test to support the clinical assessment. Treatment will be determined in line with current NICE guidelines for uncomplicated lower UTIs.
