Understanding Sinusitis

Sinusitis is an inflammation of the sinuses, the air-filled cavities located around the nose, cheeks, and forehead. It most commonly follows a cold or upper respiratory infection and is usually caused by a viral infection. In some cases, bacteria can also be responsible, particularly when symptoms have persisted or worsened after an initial period of improvement.

Acute sinusitis symptoms typically last up to four weeks, though in some cases they can persist for up to twelve weeks. The majority of cases resolve on their own with self-care, but in some circumstances, antibiotic treatment may be appropriate. What does sinusitis feel like?

Sinusitis typically causes pain or pressure around the cheeks, forehead, or behind the eyes. This is often accompanied by a blocked or runny nose, reduced sense of smell, a high temperature, and a general feeling of being unwell. Toothache in the upper teeth is also common as the sinuses sit close to the roots of these teeth.

How is sinusitis different from a cold?

A cold usually improves within seven to ten days. Sinusitis symptoms last longer, often over ten days, and tend to involve more prominent facial pain and pressure. Symptoms that worsen after an initial period of improvement, sometimes called a double sickening pattern, can suggest a bacterial secondary infection.

Do I need antibiotics for sinusitis?

Most sinusitis is caused by a virus, which antibiotics cannot treat. However, if your pharmacist assesses that your symptoms are consistent with a bacterial infection, antibiotics may be prescribed under the NHS Pharmacy First pathway. This decision is made on clinical grounds and follows current NICE guidelines.

Can children be seen for sinusitis at the pharmacy?

Yes. The NHS Pharmacy First sinusitis pathway is available for anyone aged 12 and over. Children under 12 with sinusitis symptoms should be seen by a GP.

Will sinusitis go away on its own?

In most cases, yes. Viral sinusitis usually clears up within two to three weeks with rest, fluids, and over-the-counter symptom relief. If symptoms persist beyond ten days, are severe, or keep returning, a pharmacist assessment can help determine whether further treatment is needed.

Seek urgent help if: You develop a severe headache, a stiff neck, swelling or redness around the eyes, vision changes, confusion, or a high fever that is not controlled with paracetamol. These may be signs of a serious complication and require emergency assessment.

Who Is Eligible for Pharmacy First Sinusitis Treatment?

Sinusitis is one of seven conditions covered by the NHS Pharmacy First service. The service is free and available without a GP referral.
